MyHealth Portal for Southeastern Medical Center

Southeastern Med MyHealth Portal

This interactive web portal empowers all Southeastern Medical Center patients and their families to take a more active role in their care by providing easy, secure access to health information. All of the information contained in the MyHealth Portal comes from your Southeastern Med Electronic Health Record. This ensures that you have access to the most accurate, up-to-date information possible.

Southeastern Med MyHealth Portal allows you to:

  • View laboratory results
  • Find a list of current medications, allergies and health conditions
  • View upcoming appointments
  • Download medical records or send medical records to another provider
  • View past hospital visit history
  • Update personal information
  • View or print discharge summary
  • Use Mobile App for easier access. Go to your phone’s app store and search for “MEDITECH MHealth”

First time users of the Southeastern Med MyHealth Portal must fill out a request for access. Once the form has been received and processed, you will receive an email detailing your next steps to verify your identity. Once your account has been confirmed you will be choose your username and password.

Processing of new accounts normally takes 48 business hours once we receive the completed form.

* If a proxy is necessary for any patient over 18, a healthcare power of attorney document will need to be on file in our Medical Records department.

Superior Med Physician Group Portal

The Superior Med Physician Group Portal provides our patients with secure online access to portions of your personal medical record, as well as an easy and convenient way to communicate with the practice through a secure internet connection. Think of it as an online account for your doctor’s office, similar to online banking accounts.

With Superior Med Physician Group Portal you will be able to:

  • Send and receive non-urgent messages and information from your doctor’s office, including test results and educational material

  • Easily notify the practice of changes to your personal information, including phone number, address, and insurance changes

  • Request a refill for a medication prescribed by a provider at the practice

  • View upcoming and previous appointments

MyHealth Portal FAQ

Some of my information is incorrect. How do I correct this?

If you notice an error in your personal information, such as address and phone number, please update your profile by clicking on the profile button/link on the home page and then clicking on the Update Profile Link located on the right-hand side of the screen. If you see information in your medical record that is incorrect, please notify the Medical Records department at (740) 439-8651.

What if I need technical support?

Please notify the MyHealth Support department at (740) 435-2100 or at myhealth@seormc.org.

How do I communicate my medical concerns?

MyHealth should not be used to communicate medical concerns to your doctor. IF YOU ARE EXPERIENCING AN URGENT MEDICAL PROBLEM, PLEASE CALL YOUR PHYSICIAN’S OFFICE OR CALL 911.

How do I see my test results?

Test results and reports are both available by clicking on the Health Record button on the home page. Once you click on the Health Record link you will be taken to the Health Record page where you will be able to choose the Results link on the right of the screen. This link will take you to your lab results and will display reports for any report we may have that you will have access to.

I can’t find my reports, what do I do?

list of available reports from within the portal is located at the end of the FAQ section. If you need a copy of a report that is not available in the portal or before it is available on the MyHealth patient portal please visit the Medical Records Department at Southeastern Medical Center Monday - Friday 8 AM to 5 PM or call (740) 439-8651.

How long does it take to see my Report(s)?

Reports are available 72 hours after the physician has reviewed and signed the report. If you need a copy before it is available in the portal please visit the Medical Records Department at Southeastern Medical Center Monday - Friday 8 AM to 5 PM or call (740) 439-8651.

How do I get the complete set of my medical records?

You can obtain a copy of your complete medical record by contacting the Medical Records department at (740) 439-8651.

How can I update my email address in MyHealth?

To update your email information simply click on the preferences link at the bottom of the MyHealth home page.

What information can I see in MyHealth?

MyHealth provides information from your medical record such as laboratory and radiology test results, medications, upcoming appointments, past visits, allergies, medical history, and current health issues. MyHealth also provides the opportunity to print a Health Summary for your records.

Is there a fee to use MyHealth Portal?

MyHealth is a FREE service offered to our patients.

Is MyHealth secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through personal IDs and passwords. MyHealth uses secure encryption technology to keep your records safe. All MyHealth messaging is done while you are securely logged on to our website.

Can I also see my family member’s health records?

Yes, this is called Proxy Access. This allows a parent or legal guardian to log in to their personal MyHealthPortal for their child or parent. Please note that when the account holder is between ages 13 and 17, we require the designated proxy to be filled out and signed by both parties first in order to proceed with MyHealthPortal access. What if I need more information than is available through MyHealth?

If you need additional information than that provided through the patient portal, please contact the Medical Records department at (740) 439-8651 or send an email to medicalrecords@seormc.org.

How can I sign up?

Please fill out the MyHealth Portal Request Access Form to sign up or to request an account. Once the form has been received and processed, you should receive an email detailing next steps. Processing normally takes 48 business hours.
